At our Midland Park dental office, we are passionate about helping patients of all ages and stages get the high-quality dental they deserve at an affordable price. That's why we are pleased to offer our Care Plus Program (CPP).
This program is an annual savings plan for families and individuals to receive quality dental care at significantly reduced prices. It's an affordable plan for those who don't have access to dental insurance.

Healthy Adult Program
$551/year ($738 value)
- Ages 13 and over
- 2 hygiene visits (exams and prophylaxis) per year
- 1 set of 6 cavity-checking X-rays per year
- Fluoride 2x/year
- Oral cancer screenings
- No charge emergency exams
- 20% discount on all other treatment (Invisalign, teeth whitening and electric toothbrushes not included)
- Comprehensive exam discounted at 20%
Children’s Program
$425/year ($585 value)
- Ages 12 and under
- 2 hygiene visits (exams and prophylaxis) per year
- 1 set of 4 bitewing X-rays
- Fluoride 2x/year
- No charge emergency exams
- 20% discount on all other treatment (Invisalign, teeth whitening and electric toothbrushes not included)
- Comprehensive exam discounted at 20%
Periodontal Program
$840/year ($1144 value)
- 3 periodontal visits/year (includes 3 periodontal maintenance visits and 2 periodic exams)
- 1 set of 6 cavity-checking X-rays per year
- Fluoride 2x/year
- Oral cancer screenings
- No charge emergency exams
- 20% discount on all other treatment (Invisalign, teeth whitening and electric toothbrushes not included)
- Comprehensive exam discounted at 20%
You're Covered!
- CPP can only be used at our practice.
- Plan duration is 1 year from registration.
- Membership fees are payable at time of registration and are non-refundable.
- Payment for treatment is due at time of service.
- When an additional payment plan is used (ie Care Credit), your CPP rate will be reduced by 10%.
- Family members who sign up after the initial plan member will be given a 10% signup discount on membership fee.
CPP Benefits Include
- A 20% discount on all fees*at MPFD in year 1
- Up to 3 free dental hygiene visits** per year
- Emergency exams are free (excludes treatment)
- Discount increases to 25% after year 1
*Orthodontic treatment, teeth whitening and electric toothbrushes are not included
**New Patient Exam is not included and will be reduced under normal terms
